“Sur” carries deep meaning in both Hebrew (צוּר) and Sanskrit (सुर), symbolizing strength, vitality, and well-being. It reflects our commitment to building lasting foundations for health—integrating body, mind, and spirit.

At Sur , we’re reimagining what it means to care for the whole person. As board-certified emergency medicine physicians, we've treated tens of thousands of patients in ERs across the country—and we've seen firsthand the failings of our healthcare system. Too many people can't get access to timely, physician-led care. Instead, they’re left to navigate chronic disease, side effects from one-size-fits-all treatments, and confusing advice from non-medical providers. That’s why we created Sur: to deliver evidence-based, physician-led care with a focus on longevity, metabolic health, and precision medicine. Our patients build lasting relationships with us, certified coaches, and a team committed to helping them live better, longer.

Umang Jain, MD

Co-Founder, Sur Health

Dr. Umang Jain is a board-certified Emergency Medicine physician and co-founder of Sur Health. He previously served as Department Chair and Medical Director at a large Level 2 trauma center outside of Chicago. A Boston native, he earned his medical degree from Northwestern’s Feinberg School of Medicine and completed his residency at the University of Pennsylvania.

Dr. Jain is a member of the American College of Emergency Physicians (ACEP), the American Association of Physicians of Indian Origin (AAPI), and the American Academy of Anti-Aging Medicine (A4M), with a special interest in hormone replacement therapy. He delivered a TEDx talk on emergency response. His background spans value-based care, digital health, and population health innovation, with previous experience at Oak Street Health and the Institute for Healthcare Improvement. Dr. Jain lives in Wicker Park, Chicago, with his wife.

Scott Samlan, MD

Co-Founder, Sur Health

Dr. Scott Samlan is a board-certified Emergency Medicine physician and co-founder of Sur Health. He currently serves as Chief Medical Officer at Northwest Indiana ER & Hospital. With more than two decades of clinical leadership experience, Dr. Samlan brings a forward-thinking, whole-person approach to health and longevity.

A member of the American Menopause Society and a Fellow of the American College of Emergency Physicians, Dr. Samlan focuses on evidence-based hormone therapy, nutrition, sleep science, and other core pillars of healthspan optimization. His work has been featured nationally on CNN, Fox News, ABC, and CBS This Morning, and he’s been repeatedly recognized for outstanding patient satisfaction. Dr. Samlan lives in Highland Park, Illinois, with his wife, two children, and his puppy.

Jennifer Lopez, CPT, CES, PNLI

Jennifer Lopez is a certified personal trainer (CPT), corrective exercise specialist (CES), and Precision Nutrition Coach (PNL1) with over a decade of experience in fitness and lifestyle coaching. Since 2013, she has worked as a personal trainer and group fitness instructor and is currently a co-owner of Pulse Boxing and Fitness. Through years of hands-on coaching, Jennifer found that clients who focused on nutrition achieved the most transformative and lasting results, leading her to specialize in healthy behavior change and personalized nutrition coaching. At Sur, she empowers clients with daily tools for longevity, guiding them toward sustainable success with the added benefit of physician oversight and access to advanced medical-grade treatments.
